Office hours: 8:00 - 4:30 Monday through Thursday.  8:00 - 11:30 on Friday.

One of our physicians is always on call and available for emergencies. If you have an emergency after hours, please call 231-737-3879.  Our physician's answering service will take your name, number and nature of the problem and relay this information to our physician on call.  He will return your call as soon a possible.

Referrals:
You do not need to be referred here by another physician.  Self-referrals are always welcome.  When you are referred for care by another physician a report will be sent to that physician upon completion of your visit detailing the course of action to be taken in your care.  We will also keep them informed at timely intervals during the course of your treatment.

Prescriptions:
For your prescription refills, please call our office before 4:00 p.m. during office hours so we have the necessary time to review your chart.  Before you call, please have available the name of your pharmacy, telephone number, name of the medication and dosage.

Records and Privacy:
Your medical records, created in this office, belong to this office.  Information will not be provided to a third party (non-referring physician, attorney, insurance company or government agency) unless we have written authorization from you.  If you wish information about your condition to be provided to a third party, they should request the information in writing and provide us with your written and signed authorization.  A processing fee is charged to cover the physician's time to review your record, preparation of the narrative summary, if requested, as well as the staff's time.  You are responsible for this payment if the third party refuses to pay for this service.  Information to any physician will be issued without a service fee.  There is a required pre-payment fee for the completion of disability, FMLA and AFLAC forms.
